New Hire Documents

All new hire employees at the County of Door must view and complete the following documents:

Form I9 - Section 1 of the I9 form must be completed and turned into Human Resources on or before your first day. This form must be submitted with proper verification (one item from List A OR one item from both List B and List C). Acceptable documents for verification can be found on page two (2) of the Form I9.

Evidence of Auto Insurance Sign Off Form - The Evidence of Auto Insurance Sign Off form must be completed and submitted to Human Resources on or before your first day. This form must be submitted with a copy of your auto insurance declaration page showing the coverage amounts you have meet the County's minimum requirements:

$100,000 per person
$300,000 per accident Bodily Injury
$50,000 per accident Property Damage; or
$300,000 Combined Single Limit
